If you are diagnosed with diabetes, a team of health professionals will be here to help support you in managing your condition – your doctor, our Diabetes Educator and other allied health practitioners such as a dietitian and a podiatrist.

Your doctor and Diabetes Educator will conduct a thorough assessment to determine your personal needs. This will include:

  • Blood tests
  • Foot checks
  • Blood Pressure checks
  • Body Mass Index Readings
  • Kidney Health checks
  • Cholesterol tests
  • Medication reviews
  • Recommend regular eye checks

Our Diabetes Educator will then work with you to help you understand your condition and the treatment options available. Identifying ways that you can help manage your condition yourself by making informed health and lifestyle choices will be an important part of the education process.

This will include the following:

  • Making appropriate food choices
  • Incorporating physical activity into daily life
  • Monitoring blood glucose and using the results to improve diabetes management
  • Using medications for therapeutic effectiveness
  • Problem solving for high and low blood glucose levels and for sick days
  • Reducing the risk of chronic complications
  • Adapting to work, family and social situations
  • Fees
